To print the athletic physical packet for Grades 7-12 for the 2012-2013 school year please click below.

Parents please be aware that if you need a copy of your childs physical for this summer you must get a copy before the school year ends.

Also, as you get your physicals over the summer, please turn them in to the athletic trainer. If the athletic trainer is not here, you may leave it in their box or turn it into the athletic director.
